11/18/2023 EnchantaVaughn Approved Behavior: Client expresses gratitude and compassion toward her counselor for helping her and being there for her when sheâs going through ups and downs. Client exhibits high self-esteem. Client was inquired to share something that she has learned today. Client demonstrated responsible behaviors by sitting properly and waiting patiently for the next activity. Client displayed a high level of participation in the session and is supported for her cooperation. Client is observed focused and on task. Client expresses herself in an assertive but confident way when it came to her accomplishments and was supportive to mom when she needed her help. Behavior: Client has shown a lot of improvement with dealing with her symptoms and building a healthy relationship with her family. Client negative symptoms have stabilized, she has begun to cope with all stressors, as well as, decreasing her impulsive behaviors while in the home. Client displayed appropriate behaviors. Client participated in individual and family activities without getting frustrated. Behavior: Client communicated the positive reinforcement that was provided by in-home staff. Client is seen helping others and cleaning up behind herself. Client exhibited good social behaviors, interacting well with others and demonstrating that she can behave and do what she needs to do. Intervention: QMHP provided age appropriate understanding of her mental illness and encouraged her to utilize her coping strategies during social and non-social environments. QMHP offered to help client with processing and to give her one on one to understand what it is that she is supposed to understand. QMHP administered psycho educational activity with client and assisted when needed. QMHP provided feedback regarding client inquiries. QMHP commended client on utilizing coping strategies to increase her understanding of feelings and emotions. Intervention: QMHP continued to elaborate on clientâs stressors. QMHP commended client for her participation and honesty. QMHP processed client positive behaviors. QMHP continued to monitor and receive an update on clientâs relationship with mother and compliance to duties and expectations for client in the home and in the community. Intervention: QMHP encouraged client to use her strategies and skills with peers, family, and health professionals to support her with managing symptoms associated with ADHD. QMHP reiterated the importance of positive thinking and exercising her coping skills. QMHP supported client involvement in in-home counseling. Response: Client was receptive to the interventions, as evidenced by her cheerful expressions. Client shook her head in understanding. Client said okay. Client agreed to complete 3 tasks at home daily to keep herself busy and out of trouble. Mother praised client for being on her best behavior. Client contributed to discussion and shared her personal expectations and thoughts about her daily experiences in each session. Client displayed good behaviors, waiting for her turn to speak, using her words appropriately, saying please and thank you and giving her counselor great eye contact. Response: Client said she felt good and wanted to step outside to get some fresh air. Client verbalizes positive feelings about herself at this time. Client smiled and hugged QMHP for being there for her when she needed it most. Client displayed positive interaction and was supported with building healthy relationship with sibling and peers. Response: Client implemented the positive reinforcement provided by her mother and aunts and was able to stay motivated and enjoy her session. Client smiled and thanked QMHP for the extra support and for going above and beyond to help her family during this stressful time.
